Due to circumstances beyond my control, I am sorry to announce that I will no longer be practicing at Hays Women’s Health effective October 31, 2013. I will continue to see scheduled patients until Friday, October 25, 2013 and will be available to assist with any questions or the transitioning of your care through October 31, 2013 at (512) 268-0286.
Since you may require further medical treatment and to avoid any lapse in care, I recommend you find a new physician as soon as possible. You may continue your care with Hays Women’s Health or we can assist you with transfer of care to another local provider.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate with your written consent. If you choose to see a provider other than Hays Women’s Health, please contact Hays Women’s Health for an authorization form and assistance with transferring your records. Until then, your records will remain on file at Hays Women’s Health.
I will sincerely miss having you as a patient and am truly sorry for any inconvenience this change will cause you. It has been an honor to provide your obstetric and/or gynecologic care over the past three years, and I thank you for the privilege of entrusting your health to me and to Hays Women’s Health. I applaud your efforts to make healthy choices in your life, and I encourage you to continue to take good care of yourself – emotionally, physically, mentally and spiritually.
